Delivery of the project will soon be coming to an end and we will have achieved out set targets. The overall project outcomes are: Outcome 1 Disadvantaged residents better understand how to prevent ill health for themselves and their families Outcome 2 800 people are better managing their existing/ emerging health conditions Outcome 3 Disadvantaged residents will access local information and services to support individual and family wellbeing Outcome 4 Residents in disadvantaged communities are more able to support family, friends, and community members Outcome 5 Health and digital literacy is supported and strengthened in Sandwell services
