Home From Hospital Care provides a sitting-in service to support carers to take a break once a week in order to, for example, pursue their own interests, have a rest, attend medical appointments or go shopping. This service is for Sandwell residents who have been recently admitted to City Hospital and their carers, as well as all Birmingham residents who have been admitted to Birmingham residents. The service is for approximately six weeks. A coordinator or volunteer will visit the adult and their carer at home once a weak for between one and two hours to sit with the person being cared for and to allow the carer to take a break. The carer is able to use this time in whichever way they feel will be most beneficial to them. The coordinator or volunteer visiting can also give emotional support, guidance, information and signposting where requested. This service helps to prevent carer breakdown and sustain the caring relationship as well as improve the health and wellbeing of carers.

About the organisation

Home From Hospital Care is a charity providing aftercare for people who have recently been discharged from hospital. We support Birmingham residents in Birmingham Hospitals and, since January 2019, have been supporting Sandwell residents who have been admitted to Hospitals or other health establishments based in Birmingham. The charity was established in 1972 and aimed to enhance the safety of hospital discharges. Today, we are an essential part of the discharge planning process. We work to optimise recovery, reduce loneliness and isolation and to enhance wellbeing. Our support significantly reduces the risk of re-admission to hospital. We provide a range of support including befriending, support with shopping, emotional support, benefits advice, support to attend medical appointments, support with correspondence and form filling, a sitting-in service for carers and a cleaning and handy-man service. The service is for six weeks and it is free. An illness or injury and a hospital admission can be a difficult and life changing experience. Many people face this alone, or with little support. Having someone there to provide practical support, listen, comfort and reassure can transform. Volunteers help us deliver the service and are most commonly involved in befriending and shopping. At the end of the period of support, the volunteer can take a break, or be matched with another service user. It is important that volunteers commit to the six-week period so that the support provided is reliable and consistent.
