The Dementia Navigator Service is here to help you and tour family before and after being diagnosed with dementia. The Dementia Navigator service is delivered by social enterprise Agewell Dementia Pathfinders and Crossroads, which supports older people across Sandwell in the early stages of dementia . Seeking and receiving a diagnosis of dementia can be a frightening time for both the person themselves and their family. Our aims are to: Support you in seeking a diagnosis Be there to provide the advice and support you need to take it all in Guide you and your family through what can be done now to better prepare for the future Encourage you and your carers to take part in enjoyable activities in the community Help you make the most of your life after diagnosis For further information please contact us on 0121 553 6483

About the organisation

The saying goes that 'giant oak trees from tiny acorns grow', and that's certainly the case with Agewell. In 1998, the organisation started life as the vision of a small group of older Sandwell residents. Now, the Agewell oak tree is over 10 years old, the roots are well established and its branches reach out to influence many areas affecting older people's quality of life - equality, housing, health, safety, transport and many more. It is a strategic voice for older people and is a well-respected organisation which is making a real and lasting impact on the lives of local people.
