The overall aim of Reach and Reconnect project is to include and improve the health and wellbeing and life chances of residents in some of Sandwell’s most disadvantaged communities. We achieve this by helping residents with the following: • Helping residents better understand how to prevent ill health for themselves and their families. • Residents will be better equipped to manage their existing/ emerging health conditions. • Disadvantaged residents will access local information and services to support individual and family wellbeing • Residents in disadvantaged communities are more able to support family, friends, and community members. • Health and digital literacy is supported and strengthened in Sandwell services.
